Shortening AV delay terminates all pacing-facilitated AVNRT episodes in a dual-chamber pacemaker patient.
Case Report (n=1)
Individualized pacemaker programming, specifically shortening the AV delay, can effectively diagnose and terminate pacing-facilitated AVNRT.
May allow noninvasive termination of pacing-facilitated AVNRT via AV delay adjustment; hypothesis-generating and requires prospective validation before broader adoption.
A dual-chamber pacemaker patient with sick sinus syndrome presented with a slow, long-RP' tachycardia. Device interrogation revealed that a spontaneous PVC reset the tachycardia, excluding atrial tachycardia. A subsequent atrial paced beat advanced the next QRS, excluding junctional tachycardia and implicating the slow pathway in the reentrant circuit. Diagnosis of AVNRT was confirmed. Shortening the AV delay terminated all episodes. This case demonstrates the diagnostic utility of pacemaker algorithms and the value of individualized programming.

Liu et al. (2026) conducted a case report in Sick sinus syndrome, AVNRT (n=1). Individualized pacemaker programming (shortening the AV delay) was evaluated on Termination of tachycardia episodes. Shortening the AV delay in a dual-chamber pacemaker patient with sick sinus syndrome successfully terminated all episodes of pacing-facilitated AVNRT.
