Surgical treatment of 48 neglected hip fractures utilized corrective osteotomy for malunions, total hip arthroplasty for older patients, and valgus osteotomy for younger patients with nonunions.
Observational (n=54)
In a series of neglected hip fractures, valgus osteotomy was identified as the best procedure for younger patients with femoral neck nonunions.
May support valgus osteotomy for younger patients with neglected femoral neck nonunions; hypothesis-generating case series leaves open need for controlled trials.
An untreated fracture of the hip will usually present as a nonunion or a malunion and can present a great challenge to the surgeon. We present our experience with 54 neglected hip fractures, of which 48 had surgical treatment. Malunions were treated with corrective osteotomy. Total hip arthroplasty was used for the older patients with femoral neck nonunions. The younger patients with femoral neck nonunion represented the most difficult problem, and we found valgus osteotomy to be the best procedure for this group.

Lifeso et al. (1990) conducted an observational in neglected hip fractures (n=54). Surgical treatment was evaluated. Surgical treatment of 48 neglected hip fractures utilized corrective osteotomy for malunions, total hip arthroplasty for older patients, and valgus osteotomy for younger patients with nonunions.
