Key result
Surgical MVR for acute MR precedes fatal hemodynamic deterioration driven by unrecognized cardiac amyloidosis.
Why the study?
Cardiac amyloidosis is often underdiagnosed or recognized at advanced stages, and presents atypically with acute mitral regurgitation from chordae tendineae rupture in young patients.
Population
A 22-year-old man presenting with acute severe mitral regurgitation secondary to posterior mitral leaflet chordae tendineae rupture
Design

Undiagnosed amyloidosis may cause fatal post-MVR deterioration in young acute MR; case report leaves open role of preoperative screening.
Case Report (n=1)
Cardiac amyloidosis can rarely present as acute severe mitral regurgitation due to chordae tendineae rupture in young patients, leading to rapid deterioration.
Aguilar-López et al. (2022) conducted a case report in Cardiac amyloidosis presenting as acute severe mitral regurgitation (n=1). Surgical mitral valve replacement was evaluated. Surgical mitral valve replacement in a 22-year-old man with acute mitral regurgitation was followed by fatal hemodynamic deterioration, with biopsy later revealing cardiac amyloidosis.
