Bolus water drinking reduces SBP drops in ~56% of older adults with orthostatic hypotension, beating alternatives.
Why the study?
Do nonpharmacologic interventions such as bolus water drinking improve systolic blood pressure drop in older people with orthostatic hypotension?
Do nonpharmacologic interventions such as bolus water drinking improve systolic blood pressure drop in older people with orthostatic hypotension?
Bolus water drinking is an efficacious first-line nonpharmacologic intervention for improving systolic blood pressure drop in older patients with orthostatic hypotension, whereas compression stockings are ineffective.

Supports bolus water drinking as potential first-line option in older adults with orthostatic hypotension; leaves open confirmation in randomized trials before practice change.
Newton et al. (2018) studied Orthostatic hypotension secondary to aging (n=25). Bolus water drinking vs. Compression stockings, abdominal compression, and physical countermaneuvers was evaluated on Response rate (proportion of participants whose systolic blood pressure drop improved by ≥10 mm Hg) (95% CI 36.7-74.2). Bolus water drinking improved systolic blood pressure drop by ≥10 mm Hg in 56% (95% CI 36.7-74.2) of older adults with orthostatic hypotension, superior to other interventions.
