Key result
Cilnidipine is linked to greater SBP reduction and improved renal outcomes versus telmisartan in CKD.
Why the study?
Hypertension is a common comorbidity in CKD that accelerates disease progression and increases cardiovascular risk, prompting a comparison of the effectiveness and safety of telmisartan versus cilnidipine.
Does cilnidipine 10 mg once daily improve blood pressure and renal outcomes compared to telmisartan 40 mg once daily in patients with hypertensive chronic kidney disease?

Does cilnidipine 10 mg once daily improve blood pressure and renal outcomes compared to telmisartan 40 mg once daily in patients with hypertensive chronic kidney disease?
Cilnidipine 10 mg daily may be more effective than telmisartan 40 mg daily in lowering systolic blood pressure and improving renal outcomes in patients with hypertensive CKD.
May support cilnidipine preference in hypertensive CKD; leaves open need for randomized confirmation.
Background: Hypertension is a common comorbidity in patients with chronic kidney disease (CKD), significantly contributing to disease progression and higher cardiovascular risk. Aims and Objectives: This study aimed to compare the antihypertensive effectiveness, kidney-protective effects, and safety profiles of telmisartan and cilnidipine in patients with hypertensive CKD. Materials and Methods: A prospective observational study was conducted over a 6-month period at Sree Shakthi Nursing Home in Yemmiganur. A total of 100 CKD patients with hypertension were enrolled and divided into two groups: Group A received cilnidipine 10 mg once daily, and Group B received telmisartan 40 mg once daily. Outcomes measured included changes in blood pressure, renal function parameters (serum creatinine, urea, uric acid, and proteinuria), and the incidence of adverse effects. Results: Both groups showed significant reductions in blood pressure and improvements in renal function markers. However, the cilnidipine group experienced a more notable decrease in systolic blood pressure, serum creatinine levels, and proteinuria compared to the telmisartan group. Additionally, fewer adverse effects, such as dizziness and fatigue, were reported in the cilnidipine group. Conclusion: Cilnidipine proved to be more effective than telmisartan in lowering systolic blood pressure and enhancing renal outcomes in hypertensive CKD patients, with a better safety profile. These findings suggest that cilnidipine may be a preferred treatment option for this population, although larger randomized studies are needed to confirm these results.

Uppara et al. (2025) conducted an observational in Hypertensive chronic kidney disease (n=100). Cilnidipine vs. Telmisartan 40 mg once daily was evaluated on Changes in blood pressure, renal function parameters (serum creatinine, urea, uric acid, and proteinuria), and the incidence of adverse effects. Cilnidipine was more effective than telmisartan in lowering systolic blood pressure and enhancing renal outcomes in hypertensive CKD patients, with fewer adverse effects.
