Emergency partial ileectomy fails to prevent fatal peritonitis from cholesterol crystal embolization-induced intestinal perforations.
Why the study?
Small intestinal perforation caused by cholesterol crystal embolization without obvious triggers is rare.
Case Report (n=1)
This case highlights that cholesterol crystal embolization can rarely cause multiple small intestinal perforations even in the absence of typical triggers such as vascular surgery or anticoagulation.
Cholesterol embolization merits consideration in unexplained intestinal perforations without triggers; single case leaves open optimal management strategies.

Sato et al. (2022) conducted a case report in Multiple small intestinal perforations caused by cholesterol crystal embolization (n=1). Emergency surgery (partial ileectomy) was evaluated on Clinical outcome. An 88-year-old woman with multiple small intestinal perforations caused by cholesterol crystal embolization underwent partial ileectomy but died of peritonitis on postoperative day 9.
