Mitral balloon valvuloplasty is complicated by a threadlike right atrial thrombus in a mitral stenosis case.
Why the study?
Balloon valvuloplasty avoids thoracotomy for mitral stenosis but carries potential complications.

Mitral balloon valvuloplasty can be complicated by the development of an in situ atrial thrombus, which can be detected by transesophageal echocardiography.
May warrant TEE vigilance for rare thrombus during mitral valvuloplasty; hypothesis-generating case report leaves incidence and prevention open.
Balloon valvuloplasty as a treatment for mitral stenosis may avoid thoracetomy but obviously is not without potential for complications.i, 2 We recently evaluated a patient undergoing mitral balloon valvuloplasty by transesophageal echocardiography and documented the unusual complication of a threadlike in situ atrial thrombus. A 29-year-old Hispanic female with breathlessness and mitral stenosis was referred to New York University Medical Center. Transesophageal echocardiography was performed before mitral balloon valvuloplasty, as is our routine practice to exclude atrial thrombi. Echocardiography showed stenosis of the mitral valve with commissural fusion. The leaflets were mobile, and no significant thickening of the subvalvular apparatus was seen. No evidence of atrial septal defect was detected by Doppler color flow imaging, and no atrial thrombus or mass was seen. Mitral balloon valvuloplasty was performed with an Inoue balloon. Biplane transesophageal echocardiography was per-
