Untreated autoimmune hypothyroidism presents with dyspnea caused by a 45g thyroid obscuring the trachea.
Case Report (n=1)
Case report of a patient with untreated autoimmune hypothyroidism presenting with compressive symptoms from an enlarged thyroid.
A 61-year-old man presented to the hospital after developing shortness of breath and throat swelling over the course of 2 weeks. He has a history of autoimmune hypothyroidism which was previously treated with levothyroxine (LT4) but was lost to follow up and has not taken LT4 in 4 years. Physical examination was remarkable for a 45g, firm thyroid that was centrally located and completely obscuring the trachea over the mid-anterior neck. CT head and neck was completed (Figure 1). Labs revealed a free t4 of 0.69ng/dL (ref 0.92-1.68ng/dL).
