Key result
RCA-guidewire and balloon-predilatation guidance enables ~100% success in radiolucent tricuspid valve-in-valve cases.
Why the study?
Transcatheter valve-in-valve implantation is a safer alternative to surgical reoperation for tricuspid bioprostheses degeneration, but fluoroscopically invisible prostheses pose challenges for accurate valve positioning.
Does an RCA guidewire combined with balloon pre-dilatation improve positioning and deployment success for transcatheter valve-in-valve implantation in fluoroscopically invisible tricuspid bioprostheses?

Does an RCA guidewire combined with balloon pre-dilatation improve positioning and deployment success for transcatheter valve-in-valve implantation in fluoroscopically invisible tricuspid bioprostheses?
The use of a right coronary artery guidewire and balloon predilatation provides an effective and reproducible adjunctive technique for the precise positioning of transcatheter valves in degenerated, fluoroscopically invisible tricuspid bioprostheses.
May facilitate valve-in-valve implantation in fluoroscopically invisible tricuspid prostheses; hypothesis-generating and requires prospective validation before broader use.
Tricuspid bioprostheses are prone to structural valve degeneration, often requiring complex interventions. Surgical reoperation carries high morbidity and mortality, whereas transcatheter valve-in-valve implantation offers a safer alternative. However, when the tricuspid prosthesis is fluoroscopically invisible, accurate valve positioning becomes challenging. This case series presents 3 such patients successfully treated using a novel adjunctive technique. A coronary guidewire was positioned in the right coronary artery as an anatomical marker for the atrioventricular groove. Balloon pre-dilatation was then performed to delineate the bioprosthetic valve's position, guiding precise deployment of the new valve. All procedures utilized Myval prostheses and were completed without major complications. Post-procedural echocardiography confirmed optimal valve function, low transvalvular gradients, and no residual regurgitation. This reproducible approach of combining RCA guidewire marking with balloon pre-dilatation offers an accurate, low-risk solution for managing fluoroscopically invisible tricuspid prostheses during valve-in-valve implantation.

Farandzha et al. (2025) conducted a case report in Severe tricuspid bioprosthesis dysfunction with fluoroscopically invisible valves (n=3). Transcatheter valve-in-valve implantation using RCA guidewire marking and balloon pre-dilatation was evaluated on Procedural success (precise valve deployment, optimal valve function with low gradients, and no residual regurgitation). The use of a right coronary artery guidewire combined with balloon pre-dilatation enabled successful, accurate deployment of transcatheter valves in all 3 patients with fluoroscopically invisible tricuspid prostheses.
