Lead Integrity Alert fails to detect ICD lead-system failure in a pacemaker-dependent patient.
Why the study?
ICD lead conductor failures cause significant morbidity and mortality, and Lead Integrity Alert was developed to provide advance warning of right ventricular lead fracture to reduce inappropriate ICD shocks.
Case Report (n=1)
This case highlights a potential limitation of the Lead Integrity Alert system in detecting ICD lead failures in pacemaker-dependent patients.
LIA may miss ICD lead failure in pacemaker-dependent patients; leaves open whether algorithm refinements are needed.
Transvenous implantable cardioverter-defibrillator (ICD) leads are prone to failure and are a significant cause of morbidity and mortality related to ICD systems.1–3 ICD lead conductor failures may result in adverse clinical consequences such as inappropriate ICD shocks and inhibition of pacing.4,5 Lead Integrity Alert (LIA; Medtronic, Inc, Minneapolis, MN) was developed to reduce inappropriate ICD shocks due to rapid oversensing by providing an advance warning on right ventricular (RV) lead fracture.

Kella et al. (2020) conducted a case report in Implantable cardioverter-defibrillator lead-system failure (n=1). Lead Integrity Alert was evaluated. Lead Integrity Alert failed to detect implantable cardioverter-defibrillator lead-system failure in a pacemaker-dependent patient.
