Transesophageal measurement using two or three extrastimuli successfully determines LA, AV node, or AP refractoriness.
Why the study?
Measuring left atrial refractoriness during electrophysiological evaluation of supraventricular arrhythmias is difficult due to seldom detectable left atrial capture on ECG or esophageal lead.
Does a transesophageal approach utilizing two or three extrastimuli allow for the measurement of left atrial refractoriness during electrophysiological evaluation of supraventricular arrhythmias?
Does a transesophageal approach utilizing two or three extrastimuli allow for the measurement of left atrial refractoriness during electrophysiological evaluation of supraventricular arrhythmias?
Utilizing two or three extrastimuli via a transesophageal approach overcomes the difficulty of detecting left atrial capture, enabling the measurement of left atrial, AV node, and accessory pathway refractoriness.
May facilitate non-invasive refractoriness assessment; leaves open validation against invasive electrophysiology studies.
During electrophysiological evaluation of supraventricular arrhythmias the transesophageal (TEEP) approach may be the first step but is limited in information available. One difficulty is in measuring left atrial refractoriness, as left atrial capture is seldom detectable either on ECG or via an esophageal lead. The problem may be eliminated and left atrial refractoriness measured via the esophagus, utilizing two or three extrastimuli to scan diastole to determine whether the atrial refractory period has been entered by the first extrastimulus. Measurement of left atrial and/or atrioventricular node or accessory pathway refractoriness then becomes possible.

Simoncelli et al. (1993) studied Supraventricular arrhythmias. Transesophageal measurement utilizing two or three extrastimuli was evaluated on Measurement of left atrial and/or atrioventricular node or accessory pathway refractoriness. Transesophageal measurement utilizing two or three extrastimuli allows for the successful determination of left atrial and atrioventricular node or accessory pathway refractoriness.
