Initial RV dysfunction in HLHS is linked to ~49% lower 18-month survival post-Norwood.
Why the study?
The relation between initial right ventricular function and later survival in patients with hypoplastic left heart syndrome has not been investigated.
Does initial right ventricular dysfunction predict early and intermediate survival in patients with hypoplastic left heart syndrome undergoing Norwood palliation?
Cohort (n=60)
Does initial right ventricular dysfunction predict early and intermediate survival in patients with hypoplastic left heart syndrome undergoing Norwood palliation?
Relative Risk: 11
Absolute Event Rate: 47% vs 93%
p-value: p=<0.005
Initial right ventricular dysfunction in patients with hypoplastic left heart syndrome strongly predicts intermediate and overall mortality after Norwood palliation, despite not impacting early stage I survival.

Initial RV function offers limited prognostic value in HLHS; leaves open identification of reliable risk factors for Norwood outcomes.
Altmann et al. (2000) conducted a cohort in Hypoplastic left heart syndrome (n=60). Initial right ventricular dysfunction vs. Initially normal right ventricular function was evaluated on Actuarial survival 18 months after Norwood surgery (RR ~11, p=<0.005). Initial right ventricular dysfunction in patients with hypoplastic left heart syndrome was associated with significantly lower 18-month survival after Norwood surgery (47% vs 93%; P<0.005).
