Off-label SAPIEN 3 treats pediatric tricuspid regurgitation via a small right thoracotomy.
Why the study?
Tricuspid regurgitation due to dysplasia in children can lead to symptomatic right heart failure requiring valve repair or replacement even with a hypoplastic tricuspid orifice.
Case Report (n=1)
No
Demonstrates the feasibility of using a transcatheter aortic valve (SAPIEN 3) for tricuspid valve replacement in a pediatric patient with a hypoplastic tricuspid annulus.
May expand options for small-annulus pediatric tricuspid regurgitation; hypothesis-generating and should not yet change practice.
Tricuspid regurgitation (TR) due to dysplasia in children may sometimes lead to gradual right heart failure after remaining asymptomatic during the first years of life. For this reason, tricuspid valve (TV) repair or replacement procedures become mandatory at the stage of symptomatic right heart failure, even when the diameter of the tricuspid orifice remains hypoplastic. We report a novel off-label use of the transcatheter aortic valve SAPIEN 3 (Edwards Lifesciences LLC, Irvine, Calif) in tricuspid position through a small right thoracotomy in a child.

Kalangos et al. (2020) conducted a case report in Tricuspid regurgitation (n=1). Transcatheter aortic valve SAPIEN 3 was evaluated. The transcatheter aortic valve SAPIEN 3 was used off-label in the tricuspid position through a small right thoracotomy in a child with tricuspid regurgitation.
