BIS monitoring detects possible reversible global cerebral hypoperfusion during distal grafting without neurologic sequelae.
Why the study?
Cerebral hypoperfusion is a possible complication during off-pump coronary artery bypass grafting and may be detected by bispectral index monitoring.
Can bispectral index (BIS) monitoring indicate cerebral hypoperfusion during off-pump coronary artery bypass grafting?

Can bispectral index (BIS) monitoring indicate cerebral hypoperfusion during off-pump coronary artery bypass grafting?
Bispectral index monitoring may serve as a useful indicator of cerebral hypoperfusion during off-pump coronary artery bypass grafting.
Does not support routine BIS use in OPCAB; leaves open its role as a hypoperfusion monitor pending prospective data.
In Brief Bradycardia and hypotension are common during off-pump coronary artery bypass grafting (OPCAB). We present a case of possible reversible global cerebral hypoperfusion during distal grafting of the left circumflex coronary artery. The bispectral index (BIS) suddenly decreased from values of 45–50 to 0 during distal grafting. Neurologic evaluation after immediate tracheal extubation in the operating room was normal and the 58 yr old patient did not suffer any neurologic sequelae. Postoperative recovery was uneventful and the patient was discharged 5 days after surgery. Cerebral hypoperfusion is a possible complication during OPCAB. BIS monitoring in OPCAB could be an indicator of cerebral hypoperfusion. IMPLICATIONS: We present a case of possible cerebral hypoperfusion during beating heart surgery as shown by processed electroencephalographic monitoring of the bispectral index.

Hemmerling et al. (2005) conducted a case report in Off-pump coronary artery bypass grafting (OPCAB) (n=1). Bispectral index (BIS) monitoring was evaluated on Bispectral index (BIS) values and neurologic sequelae. Bispectral index (BIS) monitoring detected a sudden decrease from 45-50 to 0 during distal grafting, indicating possible reversible global cerebral hypoperfusion without neurologic sequelae.
