Dicumarol increases phenytoin levels while phenytoin blunts dicumarol's anticoagulant activity.
Why the study?
Oral anticoagulants differ in their interactions with phenytoin, but the mechanisms and clinical implications of these interactions are not fully clarified.
What are the interactions between oral anticoagulants and phenytoin?
What are the interactions between oral anticoagulants and phenytoin?
Clinicians should be aware of significant pharmacokinetic interactions between specific oral anticoagulants (like dicumarol) and phenytoin, which can alter drug levels and efficacy.
May warrant agent-specific monitoring with phenytoin; leaves open differential interaction profiles across oral anticoagulants.
This article reviews the literature concerning oral anticoagulant-phenytoin interactions. Two cases involving concurrent warfarin-phenytoin administration are discussed. The medical literature supports a major interaction between dicumarol and phenytoin. Dicumarol significantly increases phenytoin levels, while the effect of phenytoin on dicumarol lessens anticoagulant activity. The available evidence indicates oral anticoagulants differ in their interactions with phenytoin. The mechanisms involved in oral anticoagulant-phenytoin interactions are also discussed.

Taylor et al. (1980) conducted a review in Oral anticoagulant-phenytoin interactions (n=2). Concurrent oral anticoagulant and phenytoin administration was evaluated. Dicumarol significantly increases phenytoin levels while phenytoin lessens dicumarol's anticoagulant activity, indicating that oral anticoagulants differ in their interactions with phenytoin.
