Key result
Morris index detects left atrial enlargement with ~96% accuracy compared to echocardiography.
Why the study?
Left atrial enlargement detection is important due to its association with increased morbidity and mortality, but the relationship between ECG criteria and echocardiographic LA volume index needed clarification.
Does electrocardiographic evidence of left atrial enlargement associate with left atrial volume index measured by 2D echocardiogram in cardiology patients?
Observational (n=80)
No
Does electrocardiographic evidence of left atrial enlargement associate with left atrial volume index measured by 2D echocardiogram in cardiology patients?
Electrocardiographic criteria for left atrial enlargement correlate significantly with echocardiographic left atrial volume index, supporting the utility of ECG as an accessible screening tool for left atrial enlargement.
May support Morris index for ECG detection of left atrial enlargement; leaves open prospective validation before clinical adoption.
Background: Left atrial enlargement detection is very important in clinical medicine and cardiology. In patients with cardiovascular diseases, it is associated with increased morbidity and mortality. Left atrial enlargement is associated with arrhythmias, thromboembolism, stroke, diastolic dysfunction. ECG assessment is noninvasive and universally available and ECHO is quantitatively assessing LA size. Aims and objectives: To find the association between electrocardiographic evidence of left atrial enlargement and Left atrial volume index by 2D echocardiogram. Methods: A descriptive observational study was conducted in 80 participants who attended as outpatient clinic or admitted in cardiology department, Madras medical college with electrocardiogram evidence of left atrial enlargement were subjected to 2D-ECHO and LA volume index (LAVI) were measured. Results: There was a significant association between electrocardiographic left atrial enlargement and echocardiographic assessment of left atrial size (LA volume index) in the study. Conclusion: A good association exists between ECG evidence of LA enlargement and ECHO evidence of LA enlargement

Vinila N Swaminathan N (2023) conducted an observational in Left atrial enlargement (n=80). Electrocardiographic criteria for left atrial enlargement vs. Echocardiographic left atrial volume index (LAVI) was evaluated on Diagnostic accuracy and agreement of ECG criteria for left atrial enlargement compared to echocardiographic left atrial volume index. The Morris index (P terminal force in lead V1 >40 mm ms) demonstrated the highest diagnostic accuracy (96.3%) and substantial agreement (Kappa 0.709) with echocardiographic left atrial volume index for detecting left atrial enlargement.
