Catheter ablation eliminates antidromic AVRT in familial HCM with zero recurrence at 3 years.
Why the study?
Wide QRS tachycardia in hypertrophic cardiomyopathy can be difficult to diagnose due to obscured delta waves and risk of overlooking antidromic atrioventricular reentrant tachycardia.
Case Report (n=1)
No
This case highlights that in patients with hypertrophic cardiomyopathy, delta waves may be masked by hypertrophy on ECG, making antidromic AVRT an easily missed cause of wide QRS tachycardia.
May support ablation feasibility for antidromic AVRT in familial HCM; leaves open need for prospective confirmation.

Shiraishi et al. (2014) conducted a case report in Familial hypertrophic cardiomyopathy and wide QRS tachycardia (n=1). Catheter ablation was evaluated on Recurrence of tachycardia. Catheter ablation of left-sided accessory pathways successfully eliminated antidromic atrioventricular reentrant tachycardia in a patient with familial hypertrophic cardiomyopathy, with no recurrence over 3 years.
