Rivaroxaban successfully manages AF in warfarin resistance without subsequent infarcts or bleeding.
Why the study?
Warfarin resistance is a rare but clinically important phenomenon that complicates anticoagulation management in patients with thrombotic diseases such as atrial fibrillation.
Case Report (n=1)
In cases of unexplained warfarin resistance, switching to a direct oral anticoagulant like rivaroxaban may be a safe and effective alternative.

Warfarin resistance in AF-related stroke may require switching agents; leaves open the value of pharmacogenetic evaluation in refractory cases.
Du et al. (2025) conducted a case report in Atrial fibrillation complicated by cerebral infarction and warfarin resistance (n=1). Warfarin replaced with rivaroxaban was evaluated on Bleeding or infarcts. A patient with atrial fibrillation and warfarin resistance who failed to achieve target INR despite 12.5 mg/day of warfarin was successfully switched to rivaroxaban with no subsequent infarcts.
