Emergency platinum coil embolization seals guidewire-induced coronary perforation on abciximab, avoiding emergency surgery.
Why the study?
Guidewire-induced coronary artery perforation during acute myocardial infarction treatment with abciximab can cause tamponade requiring emergency intervention, and optimal management strategies are needed.
Does emergency platinum coil embolization control leakage and prevent emergency surgery in a patient with guidewire-induced coronary artery perforation and tamponade during abciximab infusion?

Does emergency platinum coil embolization control leakage and prevent emergency surgery in a patient with guidewire-induced coronary artery perforation and tamponade during abciximab infusion?
Intracoronary platinum coil embolization can successfully seal guidewire-induced perforations and prevent emergency surgery, even in highly anticoagulated patients receiving glycoprotein IIb/IIIa blockers.
May offer bailout for abciximab-treated perforation; leaves open generalizability and need for prospective data.
This case describes a patient with an acute myocardial infarction treated with intravenous abciximab, primary angioplasty, rotablation, and stenting, who had a hydrophilic‐coated guidewire‐induced vessel perforation, leading to tamponade. Following successful percutaneous pericardiocentesis, leakage was controlled immediately with the delivery of a platinum coil into the perforation, thus avoiding emergency surgical intervention. In selected cases. placement of intracoronary platinum coils to seal guidewire‐induced perforations might prevent the need of emergency surgical intervention, especially in highly anticoagulated patients or in those receiving glycoprotein IIb/IIIa blockers.

Humberto Coto (1999) conducted a case report in Acute myocardial infarction with coronary artery perforation and tamponade (n=1). Platinum coil embolization was evaluated on Control of leakage and avoidance of emergency surgical intervention. Emergency platinum coil embolization successfully sealed a guidewire-induced coronary perforation and controlled leakage in a patient receiving abciximab, avoiding emergency surgical intervention.
