18F-FET PET/CT imaging shows false-positive bilateral uptake due to neurosarcoidosis in a pituitary tumor patient.
Case Report (n=1)
Neurosarcoidosis may cause false-positive 18F-FET uptake in pituitary lesions; leaves open the differential diagnosis in ambiguous sellar imaging.
We report a rare pituitary imaging pitfall on 18F-FET PET/CT in a 38-year-old man with a history of treated pulmonary sarcoidosis. MRIs demonstrated a left-sided pituitary neuroendocrine tumor (PitNET)/pituitary adenoma. 18F-FET PET/CT showed discordant bilateral uptake with higher activity on the right side. Histopathological analysis demonstrated a PitNET of corticotroph type on the left side and noncaseating granulomas consistent with sarcoidosis on the right side. Neurosarcoidosis should be considered a potential cause of false-positive 18F-FET uptake.

Merino et al. (2026) conducted a case report in Pituitary neuroendocrine tumor and neurosarcoidosis (n=1). 18F-FET PET/CT was evaluated on False-positive uptake. 18F-FET PET/CT imaging showed false-positive bilateral uptake in a patient with a left-sided pituitary neuroendocrine tumor due to right-sided neurosarcoidosis.
