Right parasternal approach enables successful mitral and tricuspid valve surgery after retrosternal gastric reconstruction.
Why the study?
Performing cardiac surgery via median sternotomy after retrosternal gastric tube reconstruction carries risks of gastric tube injury and mediastinitis, requiring alternative surgical approaches for mitral valve disease with annular calcification.
Case Report (n=1)
No
A right parasternal approach is a safe and effective alternative for mitral valve replacement in patients with a history of retrosternal gastric tube reconstruction, avoiding the risk of gastric tube injury associated with median sternotomy.
Right parasternal approach may enable safe mitral replacement after retrosternal gastric conduit; leaves open generalizability beyond this case.

Kanemitsu et al. (2010) conducted a case report in Severe mitral regurgitation with annular calcification and secondary tricuspid regurgitation (n=1). Mitral valve replacement and tricuspid ring annuloplasty through a right parasternal approach was evaluated on Surgical success and postoperative recovery. Mitral valve replacement and tricuspid annuloplasty through a right parasternal approach were successfully performed without complications in a patient with severe mitral regurgitation and annular calcification 20 years after retrosternal gastric tube reconstruction.
