Key result
Prolonged call-to-hospital arrival time linked to ~3% higher mortality upon arrival after OHCA.
Why the study?
The current status of out-of-hospital cardiac arrest (OHCA) care and changes in prehospital rescue activities in the aging regional city of Hirosaki were not well evaluated, and factors associated with outcomes at hospital arrival remained unclear.
What prehospital factors are associated with death at hospital arrival in patients with out-of-hospital cardiac arrest?
Observational (n=433)
No
What prehospital factors are associated with death at hospital arrival in patients with out-of-hospital cardiac arrest?
Witnessed arrest, shockable initial rhythm, and EMS drug administration are associated with improved survival to hospital arrival in OHCA patients, while prolonged transport time increases mortality.
Longer prehospital intervals associated with higher OHCA on-arrival mortality; leaves open whether optimizing EMS response times improves survival.
来院時死亡は覚知-病着時間延長(odds ratio[OR]1.03,95% confidence interval[CI]1.00–1.06)と相関し,目撃あり(OR 0.43,95%CI 0.27–0.70),初期心電図がショック適応波形(OR 0.09,95%CI 0.03–0.29),救急隊による薬剤投与(OR 0.53,95%CI 0.33–0.85)と逆相関した。

杉山 et al. (2026) conducted an observational in Out-of-hospital cardiac arrest (n=433). Prolonged call-to-hospital arrival time was evaluated on Death at hospital arrival (OR 1.03, 95% CI 1.00-1.06). Prolonged call-to-hospital arrival time was associated with increased odds of death at hospital arrival following out-of-hospital cardiac arrest (OR 1.03; 95% CI 1.00-1.06).
