The extracellular matrix (ECM) regulates tissue behavior through biomechanical signaling that converges on YAP and TAZ. Pathological ECM remodeling sustains mechanotransduction, contributing to cancer, fibrosis, chronic inflammation, and cardiovascular disease. This review presents an ECM-centered framework linking matrix composition and mechanics to integrin–cytoskeletal–nuclear signaling, reciprocal ECM remodeling, and disease-specific YAP/TAZ responses, while highlighting emerging mechanotherapeutic strategies and precision mechanomedicine.
