Key result
Adding a passive bystander electrode lowers defibrillation threshold by ~24% versus monofilament alone.
Why the study?
It was unknown whether a passive electrode effect decreases defibrillation threshold in multi-filament middle cardiac vein defibrillation.
Population
Twelve pigs with transvenous defibrillation coils and MCV electrodes
Comparison
MCV monofilament vs MCV mono + passive filaments
Design
Experimental animal study with three-reversal binary search DFT measurement

Porcine data show 24% DFT reduction with bystander MCV electrode; leaves open translation to human defibrillation practice.
Effect estimate: 24% reduction
Absolute Event Rate: 19.9% vs 25.9%
p-value: p=0.008
Paisey et al. (2006) studied Defibrillation (n=12). Passive bystander electrode adjacent to a monofilament electrode in the middle cardiac vein vs. Monofilament middle cardiac vein electrode alone was evaluated on Defibrillation threshold (DFT) (24% reduction, p=0.008). A passive bystander electrode adjacent to a monofilament electrode in the middle cardiac vein reduced the defibrillation threshold by 24% compared to a monofilament alone (19.9 vs 25.9 J; P=0.008).
