Key result
LAAC achieves 100% device success in persistent LAA thrombus with acceptable adverse clinical outcomes.
Why the study?
The safety and effectiveness of left atrial appendage closure in patients with persistent left atrial appendage thrombus and the role of cerebral embolic protection devices remain uncertain.
Systematic Review (n=136)

May support LAAC in select persistent-thrombus cases; hypothesis-generating and requires RCTs before practice change.
Cammalleri et al. (2015) conducted a systematic review in Atrial fibrillation with left atrial appendage thrombus (n=136). Left atrial appendage closure (LAAC) was evaluated on Adverse clinical outcomes (composite of death, stroke, systemic thromboembolism, device-related thrombus, major bleeding, peri-device leakage, TIA, and device-related complications). Left atrial appendage closure is feasible and safe in patients with persistent left atrial appendage thrombus, achieving 100% device success with an 11.8% rate of adverse clinical outcomes.
