Key result
Low-dose IV heparin is linked to an over ~50% reduction in PE after hip fracture.
Why the study?
The incidence and prevention of thromboembolism following hip fracture surgery were not fully characterized.
Comparison
Low-dose intravenous heparin vs no heparin; pneumatic and mechanical compression devices vs control
Design
Cohort study with ventilation-perfusion lung scans and contrast ascending venography

May reduce pulmonary embolism after hip fracture; leaves open randomized confirmation for deep vein thrombosis prevention.
Cohort (n=248)
Montrey et al. (1985) conducted a cohort in hip fracture (n=248). Low-dose intravenous heparin vs. nonheparin-treated patients was evaluated on deep venous thrombosis and pulmonary embolus. Low-dose intravenous heparin decreased the incidence of pulmonary embolism more than twofold in patients following hip fracture, although it did not significantly decrease deep venous thrombosis.
