Key result
Abrupt atrial rate acceleration triggers 2:1 His-Purkinje block or persistent 1:1 AV conduction.
Why the study?
The mechanisms and distinguishing features of functional 2:1 A-V block within the His-Purkinje system compared to true type II second-degree A-V block were unclear.
Population
Eight subjects with longer effective refractory period of His-Purkinje system than A-V node or atrium
Comparison
2:1 block within His-Purkinje system vs 1:1 A-V conduction
Design
Observational study

May mimic type II AV block during atrial acceleration; leaves open diagnostic criteria refinement in larger cohorts.
Observational (n=8)
Damato et al. (1973) conducted an observational in Functional 2:1 A-V Block (n=8). Abrupt acceleration of the atrial rate was evaluated on Occurrence of either 2:1 block within the His-Purkinje system or 1:1 A-V conduction. Abrupt acceleration of the atrial rate resulted in either 2:1 block within the His-Purkinje system or 1:1 A-V conduction in subjects with a prolonged His-Purkinje refractory period.
