Cardiometabolic continuum model integrates endocrine, inflammatory, and vascular pathways to support early multimodal prevention.
Why the study?
The complex interplay of endocrine, inflammatory, and vascular mechanisms in the development and progression of cardiometabolic disease requires comprehensive understanding to improve prevention and treatment strategies.
Understanding the cardiometabolic continuum as an integrated network of endocrine, inflammatory, and vascular mechanisms highlights the critical need for early, personalized, and multimodal prevention and treatment strategies.
Supports early multimodal prevention strategies; leaves open prospective validation of personalized cardiometabolic models.
The cardiometabolic continuum connects metabolic derangements, hormonal imbalance, chronic inflammation, and vascular compromise into a comprehensive model of how all of these factors work together in the development and progression of heart disease. Most notably, insulin resistance leads to the accumulation of excess body fat, excess cholesterol in the blood, hypertension, and diabetes mellitus - all of which drive persistent low-grade inflammation, endothelial cell damage, free radical generation, and neurohormonal activation. The synergy of these factors promotes the acceleration of atherosclerosis, left ventricular remodeling and ultimately heart failure. To help maintain normal function of the cardiovascular system, the endocrine organs and adipose (fat) tissue function to secrete various hormones, adipokines and inflammatory mediators. These secreted substances influence vascular function and metabolic balance. The application of advances in molecular biology and the identification of new biomarkers has improved the understanding of the complex interplay of endocrine, immune, and vascular processes that contribute to the progression of cardiometabolic disease. The thorough evaluation of inflammatory markers, metabolic biomarkers, and cardiovascular risk profiles provides a means to identify individuals who are at the highest risk of developing cardiovascular disease earlier and to develop personalized treatment plans for them. The current strategies for managing cardiometabolic disease are focused on integrating the control of metabolic risk factors (eg, diabetes, hypertension), lifestyle changes, targeted medications, and the prevention of cardiovascular complications. This new approach to managing cardiometabolic disease recognizes the cardiometabolic continuum and serves as a platform to develop individualized prevention and treatment strategies that will improve long-term cardiovascular outcomes. https://doi.org/10.52783/jchr.v16.i4.15497
