Elevated mean right atrial pressure was an independent predictor of a discontinuous intrarenal venous flow pattern (OR 1.16 per mmHg increase) in patients with heart failure across all chronic kidney disease stages.
Why the study?
Intrarenal venous flow patterns have been correlated with hemodynamic parameters in heart failure but their applicability across all CKD stages, including dialysis patients, was unclear.
Does intrarenal venous flow (IRVF) analysis accurately detect impaired hemodynamics in heart failure patients regardless of their chronic kidney disease stage?
Observational (n=228)

Intrarenal venous flow (IRVF) pattern analysis is a useful noninvasive method for detecting elevated central venous and right-sided pressures in heart failure patients, maintaining its diagnostic validity across all stages of chronic kidney disease including patients on dialysis.
May support renal venous flow assessment in HF; extends prior associations across CKD stages but remains hypothesis-generating.
). The IRVF patterns were categorized as either continuous or discontinuous. Methods Hemodynamic parameters assessed using right heart catheterization included the mean right atrial pressure (mRAP), mean pulmonary artery pressure (mPAP), mean pulmonary artery wedge pressure (mPAWP), and mean inferior vena cava pressure (mIVCP). The associations between IRVF patterns and hemodynamic parameters were analyzed, and a multivariate logistic regression analysis was performed to identify the independent predictors of discontinuous IRVF.Results Discontinuous IRVF was observed in 65 patients (29%) and it was more frequent in those with lower eGFR. Across all CKD stages, patients with discontinuous IRVF had significantly higher mRAP, mPAP, and mPAWP than those with continuous IRVF. A multivariate analysis identified mRAP, mPAP, and moderate or greater tricuspid regurgitation as independent predictors of discontinuous IRVF.Conclusion An IRVF analysis may be a useful noninvasive method for detecting impaired hemodynamics in patients with CKD, including those undergoing dialysis.

Kondo et al. (2026) conducted an observational in Heart failure and chronic kidney disease (n=228). Elevated mean right atrial pressure (per mmHg increase) vs. Reference mean right atrial pressure was evaluated on Presence of a discontinuous intrarenal venous flow (IRVF) pattern (OR 1.16, 95% CI 1.06-1.29, p=<0.001). Elevated mean right atrial pressure was an independent predictor of a discontinuous intrarenal venous flow pattern (OR 1.16 per mmHg increase) in patients with heart failure across all chronic kidney disease stages.
