EVOQUE transcatheter tricuspid valve replacement shows feasibility in carcinoid heart disease with torrential regurgitation.
Why the study?
Severe tricuspid regurgitation in carcinoid heart disease poses increased procedural risk for transcatheter edge-to-edge repair, necessitating alternative treatment approaches.
Case Report (n=1)
This case report highlights the feasibility of utilizing the transfemoral transcatheter tricuspid valve replacement (EVOQUE system) for managing torrential tricuspid regurgitation secondary to carcinoid heart disease in a high-surgical-risk patient.
TEER may be feasible in selected carcinoid patients with torrential TR; leaves open need for prospective safety and efficacy data.
Excessive fibrotic tissue deposits occur in carcinoid heart disease, typically affecting right-sided valves such as the tricuspid valve. Consequently, heart failure can arise from resultant severe tricuspid regurgitation (TR). While transcatheter edge-to-edge repair is an effective treatment modality for TR, there is increased procedural risk in patients with torrential TR, large coaptation gap, or severe leaflet tethering. An 81-year-old man with carcinoid heart disease and torrential TR presented for planned transfemoral transcatheter tricuspid valve replacement with the EVOQUE system. This case demonstrates the potential for broader implementation of the transfemoral tricuspid valve replacement strategy in patients with considerable perioperative risk.

Zlaket et al. (2026) conducted a case report in Carcinoid heart disease and torrential tricuspid regurgitation (n=1). Transfemoral transcatheter tricuspid valve replacement with the EVOQUE system was evaluated. Transfemoral transcatheter tricuspid valve replacement with the EVOQUE system demonstrated potential for feasibility in a patient with carcinoid heart disease and torrential tricuspid regurgitation.
