Combined conventional-to-sutureless repair for neonatal TAPVD achieves unobstructed anastomosis with no pulmonary hypertension at 6 months.
Why the study?
A combined conventional-to-sutureless repair approach was used due to intraoperative anatomical mismatch in supracardiac total anomalous pulmonary venous drainage in a neonate.
Case Report (n=1)
Intraoperative conversion to a sutureless technique for supracardiac total anomalous pulmonary venous drainage repair was safe and effective in a neonate with a size mismatch between the left atrium and pulmonary venous collector.
Supports safety of sutureless conversion in mismatched supracardiac TAPVD; leaves open need for prospective series before wider use.
This case describes a combined conventional-to-sutureless repair of the supracardiac form of total anomalous pulmonary venous drainage in a neonate. The diagnosis was established via echocardiography in a patient presenting with mild symptoms. Anatomically, a well-developed pulmonary venous confluence was identified, with the vertical vein draining into the innominate vein and obstruction detected at the interatrial level. Although the patient was initially scheduled for a semi-elective conventional repair, an intraoperative mismatch between the left atrium and the pulmonary venous collector necessitated a switch to a sutureless technique. Postoperative echocardiography confirmed an unobstructed anastomosis between the left atrium and the pulmonary venous confluence. The postoperative course was uneventful, and at the 6-month follow-up, the patient is doing well with no evidence of pulmonary hypertension.

A 2026 study conducted a case report in Supracardiac total anomalous pulmonary venous drainage (n=1). Combined conventional-to-sutureless repair was evaluated on Postoperative clinical and echocardiographic outcomes. Combined conventional-to-sutureless repair was successfully performed in a neonate with total anomalous pulmonary venous drainage, yielding an unobstructed anastomosis and no pulmonary hypertension at 6 months.
