STAR Apollo mapping successfully identifies non-pulmonary vein AF drivers in redo ablation cases.
Why the study?
Identifying non-pulmonary vein drivers of AF in redo catheter ablation is challenging, and integration of STAR Apollo mapping with CARTO 3 system had not been reported.
Case Report
This represents the first reported clinical integration of STAR Apollo mapping with the CARTO 3 system for identifying non-pulmonary vein drivers during redo AF ablation.
STAR Apollo-CARTO integration feasible in redo AF case; hypothesis-generating for non-PV driver targeting, needs prospective validation.
Identifying non-pulmonary vein drivers of atrial fibrillation (AF) in redo catheter ablation is challenging. Stochastic trajectory of ranked signals (STAR) Apollo Mapping identifies Repetitive Patterns of Activation (RPA) and Early Sites of Activation (ESA) during AF. Unipolar electrograms are analyzed to calculate wavefront trajectories identifying regions that may be origins of AF. We report the first integration of STAR Apollo mapping with the CARTO TM 3 system (Johnson & Johnson Med Tech) in redo AF patients following prior pulmonary vein isolation (PVI).

Matthew McKillop (2026) conducted a case report in Atrial fibrillation. STAR Apollo mapping system integrated with the CARTO 3 system was evaluated. STAR Apollo mapping integrated with the CARTO 3 system was successfully used to identify non-pulmonary vein drivers of atrial fibrillation in a first reported case series of redo ablation patients.
