Pediatric ESRD on hemodialysis is linked to reduced RV function, which correlates with elevated hs-cTnI.
Why the study?
Early cardiac dysfunction in children on hemodialysis for ESRD often goes unnoticed due to diagnostic limitations and nonspecific symptoms, and the utility of hs-cTnI as a biomarker in this population remains uncertain.
Does highly sensitive troponin I correlate with early structural and functional myocardial dysfunction in children with end-stage renal disease on hemodialysis?

Does highly sensitive troponin I correlate with early structural and functional myocardial dysfunction in children with end-stage renal disease on hemodialysis?
Highly sensitive troponin I is a potentially useful biomarker for detecting early subclinical right ventricular dysfunction in pediatric patients undergoing regular hemodialysis.
hs-cTnI may mark subclinical RV dysfunction in pediatric hemodialysis; hypothesis-generating, needs prospective validation before clinical use.
Background: When it comes to children on hemodialysis for end-stage renal disease (ESRD), cardiovascular disease (CVD) is still the biggest killer. Due to diagnostic limitations and nonspecific symptoms, early cardiac dysfunction typically goes unnoticed in this population. While hs-cTnI has shown promise as a biomarker for early myocardial damage detection, its use in pediatric dialysis patients is still up in the air. Aim: To evaluate hs-cTnI and hs-CRP levels in children with ESRD on regular hemodialysis and explore their relationship with structural and functional cardiac abnormalities. Subjects and methods: From October 2023 through October 2024, 80 children from the Pediatric Nephrology and Hemodialysis Unit at Al-Zahraa Hospital, Al-Azhar University in Cairo, were the subjects of this case-control research. There were two groups: one for the patients (N=40) and another for the control subjects (N=40). Results: Statistical analysis revealed that when comparing the patient group to the control group, the following measures of right ventricular function were significantly reduced: right ventricular fractional shortening (RVFS), right ventricular global longitudinal strain (RVGLS), right ventricular ejection fraction (RVEF 2D), transannular plane systolic excursion (4D), and RVEF (4D). Each of the extremely sensitive troponin I initial samples and confirmatory samples showed a statistically significant negative correlation with RVFS. Conclusion: Children on hemodialysis may benefit from the use of the sensitive biomarker hs-cTnI for the early diagnosis of cardiac damage. Echocardiographic examination and routine monitoring of hs-cTnI can help detect subclinical cardiac dysfunction, which could lead to better cardiovascular outcomes if treated early.

Mansour et al. (2026) conducted a case-control in End-stage renal disease (ESRD) (n=80). End-stage renal disease on regular hemodialysis vs. Healthy controls was evaluated on Right ventricular function and correlation with hs-cTnI. Children with ESRD on hemodialysis exhibited significantly reduced right ventricular function compared to controls, with hs-cTnI negatively correlating with right ventricular fractional shortening.
