Why the study?
Early diagnosis and medical management of acromegaly-related heart failure may optimize cardiovascular function and reduce perioperative risks, but clinical guidance is limited.
Case Report (n=1)
Medical management of acromegaly-related heart failure can lead to significant recovery of systolic function, facilitating safe perioperative management.
This report describes a case of undiagnosed acromegaly in a 73-year-old female presenting for robotic supracervical hysterectomy. During the index operation, prolonged abdominal insufflation was not tolerated secondary to a severely depressed ejection fraction (EF) of 10% to 15%. The procedure was aborted and the patient was treated for acromegaly and heart failure. Fifteen months following treatment, her EF improved to 50% to 55%, and she underwent successful pelvic organ prolapse repair without complications. If clinical suspicion for acromegaly arises, early diagnosis and medical management may allow for optimization of cardiovascular function and reduced risks of perioperative instability.
