Novel surgical AF workflows expand treatment options through robotic, modified CryoMaze, and hybrid ablation approaches.
Why the study?
Surgical techniques and clinical considerations for atrial fibrillation treatment have significantly evolved over the past decades, necessitating an updated overview.
Highlights recent advancements in surgical techniques for atrial fibrillation, including novel workflows and perioperative considerations.
The fundamentals of effective surgical treatment for atrial fibrillation were established >30 years ago with the development of the Cox maze operation. Restoration of atrioventricular synchrony, reduction of thromboembolic stroke risk, and symptom relief have always been the primary goals of therapy. Over time, surgical techniques and clinical considerations have significantly evolved, reaching a crescendo in the past decade. Herein, we highlight novel surgical workflows-specifically the modified CryoMaze III, robotic-assisted Cox maze, nonatriotomy surgical ablation, and hybrid ablation. We also provide a focused discussion on atrial fibrillation-related dementia, tachycardia-induced heart failure, and updates in perioperative care.

Kiankhooy et al. (2026) conducted a review in Atrial Fibrillation. Evolving surgical techniques (modified CryoMaze III, robotic-assisted Cox maze, nonatriotomy surgical ablation, hybrid ablation) was evaluated. This review highlights the evolution of novel surgical workflows for atrial fibrillation, including modified CryoMaze III, robotic-assisted Cox maze, nonatriotomy surgical and hybrid ablation.
