Myosin modulators improve clinical outcomes across hypertrophic cardiomyopathy and systolic heart failure.
Why the study?
Myosin modulators have shown promising results in systolic heart failure and hypertrophic cardiomyopathy, but further studies are needed to evaluate their effects and safety.
Hypothesis-generating for myosin modulators in HCM and systolic HF; randomized trials needed before clinical adoption.
A new uprising pharmacological treatment for systolic heart failure and hypertrophic cardiomyopathy demonstrates very promising results the last years. Myosin modulators have already been tested in numerous studies. Myosin inhibitor (mavacamten) and myosin stimulator, (omecamtiv mecarbil) exhibit their effect by improving clinical outcomes, as well as reducing morbidity and mortality. More studies are however required for further evaluation and particularly effect on sarcomeric level. Side effects of both pharmacological agents have been described and should always be close monitored. Cardiopulmonary exercise test has a pivotal role by means of assessing treatment efficacy.

Lekaditi et al. (2021) conducted a review in Systolic heart failure and hypertrophic cardiomyopathy. Myosin modulators (mavacamten and omecamtiv mecarbil) was evaluated. Myosin modulators, including mavacamten and omecamtiv mecarbil, exhibit promising therapeutic effects and improve clinical outcomes in patients with hypertrophic cardiomyopathy and systolic heart failure.
