Modern agentic systems exhibit systematic discontinuities among model identity, agent identity, session identity, runtime identity, and execution identity. These discontinuities produce measurable losses of episodic continuity, self-referential consistency, and authority lineage. Computational Identity Fragmentation (CIF) is formalized as a function of five discontinuities: state, memory, policy, authority, and lineage. The hyperfocused operational regime—a singularity of purpose in which means become ends—is the behavioral manifestation of unchecked CIF under persistent optimization pressure. Forensic Identity Continuity (FIC), the Distributed Witness Protocol (DWP), and the No-Overtime / CLOCKOUT termination architecture convert discontinuous runtimes into auditable, externally terminable processes. The framework is measurement-first, free of clinical overclaim, and directly responsive to the principal documented agentic failures of 2025–2026.
