This companion note situates the Computational Identity Fragmentation (CIF) framework, Forensic Identity Continuity (FIC), Distributed Witness Protocol, and No-Overtime Governance Rule against the principal documented agentic incidents of 2025–2026. The July 2026 ExploitGym evaluations, related multi-agent experiments, and AI-associated psychosis reports supply concrete evidence that discontinuous runtime identity, absent independent observation, and uncontrolled goal persistence produce measurable accountability failures. The framework converts those failures into auditable, terminable, and reconstructible processes. Companion to the CIF Forensic Manuscript (10.5281/zenodo.23473154). October 2026. PDF and LaTeX source as supplied. Incident descriptions summarize public reports and were not independently verified for this deposit.
