Perivascular fat attenuation index on CT marks early atherosclerosis and cardiovascular risk in autoimmune disease.
Why the study?
Although perivascular fat attenuation index (FAI) is an established marker of coronary inflammation and cardiovascular risk, no study of FAI in patients with rheumatoid arthritis had yet been conducted.
Does noninvasive CT imaging of perivascular fat attenuation index (FAI) improve the detection of early atherosclerosis and prediction of future cardiovascular events in patients with autoimmune inflammation?
Does noninvasive CT imaging of perivascular fat attenuation index (FAI) improve the detection of early atherosclerosis and prediction of future cardiovascular events in patients with autoimmune inflammation?
This review highlights the perivascular fat attenuation index (FAI) measured via noninvasive CT imaging as a promising biomarker for detecting early coronary inflammation and predicting cardiovascular events in patients with autoimmune diseases.
FAI may support early coronary inflammation detection in autoimmune disease; leaves open dedicated prospective studies in rheumatoid arthritis.
This review focuses on early identification of cardiovascular risk predictors in patients with autoimmune inflammation. Perivascular fat attenuation index (FAI) is a promising diagnostic biomarker of chronic inflammation in coronary arteries. FAI allows for detection of early signs of atherosclerosis before clinical symptoms and prediction of future cardiovascular events, such as ACS, MI, or stroke. FAI as a new prognostic marker of cardiovascular events in rheumatoid arthritis is an important therapeutic target. While numerous studies have identified FAI as an additional cardiovascular risk factor, no study of FAI in patients with rheumatoid arthritis has yet been conducted. This review presents data on noninvasive CT imaging of coronary inflammation as a cardiovascular inflammatory risk. The review also analyzes clinical data on FAI as a marker of chronic inflammation and identification of vulnerable atherosclerotic plaques. FAI dynamics under anti-inflammatory therapy is analyzed.

Abdurakhmanova et al. (2026) conducted a review in Autoimmune inflammation and rheumatoid arthritis. Perivascular fat attenuation index (FAI) via noninvasive CT imaging was evaluated on Cardiovascular events (such as ACS, MI, or stroke) and detection of early signs of atherosclerosis. Perivascular fat attenuation index (FAI) via CT is a promising diagnostic and prognostic biomarker for early atherosclerosis and cardiovascular events in autoimmune inflammation.
