Key result
Semi-Fowler positioning plus pursed-lip breathing eases dyspnea and raises SpO2 to ~97% in CHF.
Why the study?
Dyspnea is a primary complaint in patients with CHF that limits daily activities and reduces quality of life, prompting the evaluation of combined Semi Fowler positioning and Pursed Lip Breathing as a non-pharmacological nursing intervention.

Does the combination of the Semi Fowler position and Pursed Lip Breathing reduce dyspnea and improve respiratory status in patients with Congestive Heart Failure?
In a descriptive nursing case study of two patients with congestive heart failure, combining the Semi Fowler position with Pursed Lip Breathing over 3 days helped reduce dyspnea, lower respiratory rate, decrease accessory muscle use, and improve oxygen saturation.
Congestive Heart Failure (CHF) is a clinical syndrome caused by the inability of the heart to pump blood adequately, which may lead to pulmonary congestion and dyspnea. Dyspnea is one of the main complaints experienced by patients with CHF and may limit their ability to perform daily activities and reduce their quality of life. One non-pharmacological approach that can be applied in nursing care to help manage this condition is a combination of the Semi Fowler position and Pursed Lip Breathing technique. This case study aimed to provide nursing care through the application of a combination of the Semi Fowler position and Pursed Lip Breathing to reduce dyspnea in patients with CHF at Dr. A. Dadi Tjokrodipo Regional Hospital, Bandar Lampung. A case study design involving two patients with CHF and dyspnea was used. Data were collected through interviews, observation, physical examination, documentation, and medical record review. The combination of the Semi Fowler position and Pursed Lip Breathing was implemented for 3 × 24 hours. The results showed that both patients experienced reduced complaints of dyspnea, decreased respiratory rate, reduced use of accessory respiratory muscles, and increased oxygen saturation, indicating an improvement in respiratory status. Therefore, the combination of the Semi Fowler position and Pursed Lip Breathing can help reduce dyspnea and improve respiratory and oxygenation status in patients with Congestive Heart Failure (CHF).

Rochmah et al. (2026) conducted a case report in Congestive Heart Failure (CHF) with dyspnea (n=2). Combination of Semi Fowler position and Pursed Lip Breathing (PLB) was evaluated on Reduction in dyspnea, respiratory rate, and improvement in oxygen saturation (SpO2). Combined Semi Fowler positioning and Pursed Lip Breathing for 3 days reduced dyspnea, lowered respiratory rate from 24-25 to 20-22 breaths/min, and increased SpO2 to 97% in two CHF patients.
