Does the semi-Fowler position improve breathing patterns and reduce dyspnea in a patient with Congestive Heart Failure?
The application of the semi-Fowler position is an effective non-pharmacological nursing intervention to relieve dyspnea and improve breathing patterns in patients with congestive heart failure.
May support semi-Fowler positioning for dyspnea relief in CHF; single-case data leaves open need for controlled trials.
Background: Congestive Heart Failure (CHF) is a condition in which the heart fails to pump blood effectively, resulting in inadequate oxygen and nutrient supply to body tissues. This condition may lead to respiratory disorders, fluid accumulation, and decreased tissue perfusion. Patients with CHF commonly experience dyspnea due to ventilation–perfusion imbalance, therefore appropriate nursing interventions are required to improve oxygenation status. Objective: This scientific paper aims to provide comprehensive nursing care for patients with cardiovascular system disorders caused by Congestive Heart Failure (CHF) through the implementation of the semi-Fowler position intervention to overcome ineffective breathing patterns. Method: The method used was descriptive analytic with a case study approach involving one client in the ICU ward of RSUD Cililin in 2026. Data were collected through interviews, observation, physical examination, documentation review, and literature study. Data were analyzed using the nursing process approach including assessment, nursing diagnosis, intervention, implementation, and evaluation. Results: The assessment results showed that the patient experienced dyspnea, somnolent consciousness, increased respiratory rate, general weakness, and signs of decreased oxygenation. Nursing implementation focused on respiratory monitoring, oxygen therapy, fluid management, health education, and application of the semi-Fowler position. Evaluation results indicated improvement in breathing patterns, reduction of dyspnea complaints, and increased patient comfort after interventions were provided. The application of the semi-Fowler position was proven to help improve lung ventilation and oxygenation in patients with CHF. Conclusion: This case study concludes that comprehensive nursing care with the implementation of the semi-Fowler position is effective in helping overcome ineffective breathing patterns in patients with Congestive Heart Failure (CHF).

Fauziyah et al. (2026) conducted a case report in Congestive Heart Failure (n=1). Semi-Fowler position was evaluated on Improvement in breathing patterns and reduction of dyspnea. Implementation of the semi-Fowler position effectively improved breathing patterns, reduced dyspnea, and increased comfort in a patient with Congestive Heart Failure.
