Diabetes mellitus is classified as a chronic metabolic disorder that causes various systemic complications, including hematological disturbances characterized by reduced hemoglobin levels. This study aimed to measure hemoglobin levels, determine erythrocyte indices specifically Mean Corpuscular Volume (MCV), Mean Corpuscular Hemoglobin (MCH), and Mean Corpuscular Hemoglobin Concentration (MCHC) and analyze the relationship between hemoglobin levels and erythrocyte indices in patients with diabetes mellitus at Dr. Iskak Regional General Hospital (RSUD), Tulungagung. The study employed an observational analytic design with a cross-sectional method, involving 35 patients with diabetes mellitus as respondents. Hemoglobin levels and erythrocyte indices were measured using an XN-1000 hematology analyzer. The results showed that reduced hemoglobin levels were observed in the majority of respondents, whereas most erythrocyte indices remained within the normal range. Spearman correlation analysis revealed no significant relationship between hemoglobin levels and MCV (r=0.191; p=0.270), MCH (r=0.254; p=0.141), or MCHC (r=0.236; p=0.173). All relationships showed weak correlations with significance values greater than 0.05. Thus, there is no significant relationship between hemoglobin levels and erythrocyte indices in patients with diabetes mellitus. The decrease in hemoglobin levels was not consistently accompanied by changes in erythrocyte indices; consequently, the majority of respondents exhibited a normocytic-normochromic erythrocyte profile.
