Stock-data discrepancies between warehouse management systems and the physical quantities of materials can reduce inventory-information reliability and disrupt warehouse operations. This study aims to identify risk events and risk agents associated with stock-data discrepancies at the warehouse of PT Paiton Operation & Maintenance Indonesia, prioritize their causes, and determine preventive actions. Warehouse activities were mapped using the Supply Chain Operations Reference (SCOR) model, while risk prioritization and mitigation were analysed through House of Risk (HOR) Phases 1 and 2. The analysis identified 44 risk events and 44 risk agents across the Plan, Source, Make, Deliver, Return, and Enable processes. HOR Phase 1 showed that repeatedly unresolved system-physical stock discrepancies (A22) had the highest Aggregate Risk Potential (4,224), followed by a disorganized warehouse layout (A1; 2,416) and unconfirmed deliveries in the system (A32; 1,854). HOR Phase 2 ranked the buddy system for stocktaking (PAk22) as the highest-priority mitigation, with an Effectiveness-to-Difficulty Ratio of 15,547, followed by item classification, identity labelling, and zoning (PAk5; 9,047), and a cut-off time for entering stocktaking results (PAk23; 6,809). These results prioritize proposed controls involving personnel, procedures, facilities, and information systems; their effects on stock accuracy have not been tested through implementation. The improvement measures proposed based on the risk mitigation analysis will be implemented at PT POMI in the subsequent implementation phase.
