Chondrosarcomas comprise a heterogeneous group of malignant lesions with characteristic cartilage matrix production. In the head and neck region, sino-nasal involvement is unusual with nasal congestion and obstruction presenting as primary symptoms. We present a case of a 64-year-old lady presenting with right-sided proptosis with decreased vision. MRI revealed a sino-nasal space occupying lesion extending into the right orbit. The patient was managed with endonasal endoscopic tumor resection and adjuvant chemotherapy. Ocular anatomy was restored and visual acuity improved post-operatively. Chondrosarcomas of paranasal sinuses pose diagnostic and therapeutic challenges. A comprehensive range of medical investigations enables timely recognition and treatment.
