Observational study highlights lipid ratios and troponin T levels in NSTEMI patients, indicating atherogenic risk.
Objective. To determine the levels of lipids and their ratios in patients with non-ST elevation myocardial infarction (NSTEMI) and different serum troponin T. Material and methods. There were 165 stable patients with NSTEMI. Standard 12-lead ECG was performed. Troponin T was analyzed using chemiluminescent immune method on microparticles upon admission and 6 hours after onset of chest pain. Serum troponin T was determined using the Cobas H 232 cardiac marker analyzer and the TNT-HS Roche Diagnostics test system. Total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoproteins (HDL-C), very low-density lipoproteins (VLDL-C), triglycerides (TG) were analyzed using enzymatic method combined with solid-phase technology. Atherogenic coefficient (AC) was calculated. Statistical analysis was carried out in STATISTICA 16 program. Results. Mean age of patients was 61.8±4.5 years, men prevailed (68.5%). ECG changes were detected in 79.4% of cases. Overweight was observed in 40.6%, obesity — in 20.6% of patients. Mean serum concentration of troponin T was 0.46±0.32 ng/ml. Two groups were distinguished depending on serum troponin T: group 1 — 58 patients with troponin T 0.05—0.09 ng/ml (mean 0.07±0.01 ng/ml), group 2 — 107 patients with troponin T 0.18—1.0 ng/ml (mean 0.67±0.18 ng/ml). Mean TC, TG, VLDL-C, TC/HDL-C, TG/HDL-C and LDL-C/HDL-C were higher in group 2 (p<0.05). AC was insignificantly high in group 2 (p=0.065). Conclusion. TC/HDL-C, TG/HDL-C and LDL-C/HDL-C ratios are significantly higher in NSTEMI patients with serum troponin T 0.67±0.18 ng/ml compared to patients with serum troponin T 0.07±0.01 ng/ml (p<0.05). Lipid parameters and their ratios are important, since they are atherogenic factors of myocardial infarction.
