Key result
HFpEF and HFrEF show decreased mitochondrial area, with HFrEF exhibiting altered BNIP3 and PGC-1α expression.
Why the study?
The FOXO3a-BNIP3 pathway alters mitochondrial dynamics and function in rodent heart failure models, but its expression and associated mitochondrial markers required evaluation in human HFpEF and HFrEF myocardium.
Population
Patients undergoing AVR, CABG, or LVAD implantation with HFpEF or HFrEF, plus normal controls
Comparison
HFpEF vs HFrEF vs normal controls
Design
Cross-sectional myocardial biopsy study

Human HF biopsy data on mitochondrial fragmentation should not change practice; extends rodent FOXO3a-BNIP3 findings to patients.
Human myocardial biopsies reveal that mitochondrial fragmentation and cristae destruction occur in HFpEF and are further accentuated in HFrEF, accompanied by distinct molecular alterations in mitochondrial dynamics and biogenesis.
Chaanine et al. (2019) studied this question. Mitochondrial area decreased to 0.71±0.08 in HFpEF and 0.54±0.06 in HFrEF, with significant increases in BNIP3 and decreases in PGC-1α and COX IV in HFrEF.
