目的:高齢者における大腸癌の術後合併症と術前プレアルブミン値との関連を明らかにし,術前プレアルブミン値の改善と効果を評価する. 方法:2018年6月から2023年3月に原発巣切除を受けた80歳以上の大腸癌患者29例を対象とし,術前プレアルブミン値と術後合併症との関係を検討した.また,術前プレアルブミン値が15mg/dL以下の患者に対して栄養介入を実施した症例でのプレアルブミン値の推移,合併症との関係を検討した. 結果:年齢(P<0.05)とプレアルブミン値(P<0.01)が独立した術後合併症のリスク因子であった.栄養介入によりプレアルブミン値が10%以上上昇した症例では,その他の低プレアルブミン値の患者に比べて術後合併症の発生率が低下し,重症度も軽減される傾向を認めた. 結論:術前プレアルブミン値が大腸癌患者の術後合併症リスクと関連し,プレアルブミン値を上昇させることで術後合併症リスクを低減できる可能性が示唆された.
