TEE LMCA peak diastolic velocity above ~85 cm/s predicts high-risk SYNTAX scores in MI patients.
Why the study?
The study aimed to correlate transthoracic and transesophageal echocardiography parameters with SYNTAX score categories in patients with coronary artery disease.
Do trans-thoracic and transesophageal echocardiography parameters accurately predict SYNTAX score categories in patients with coronary artery disease?
Do trans-thoracic and transesophageal echocardiography parameters accurately predict SYNTAX score categories in patients with coronary artery disease?
Transesophageal echocardiography parameters, such as left main coronary artery peak diastolic velocity and diameter, can reliably predict SYNTAX score categories in patients with coronary artery disease.
Context: Atherosclerosis is caused by the interplay of multiple risk factors, including traditional ones and nontraditional factors. Aortic atherosclerosis can lead to cerebral embolism (stroke or transient ischemic attack), and peripheral and visceral embolism. Techniques such as trans-thoracic echocardiography and transesophageal echocardiography (TOE) are noninvasive techniques used to evaluate arterial atherosclerosis. Our study aimed to correlate the trans-thoracic echocardiography/TOE parameters with the synergy between percutaneous coronary intervention with Taxus and cardiac surgery (SYNTAX) score (SS) categories. Materials and Methods: Our research was a cross-sectional study conducted in the cardiology inpatient department of a tertiary medical facility over 1 year. This research entailed patients undergoing acute myocardial infarction, unstable angina, chronic coronary syndrome, as well as patients who had been clinically and hemodynamically stable. Results: In our study, a total of 100 patients were included. The receiver operating characteristics curve was computed for all the trans-thoracic echocardiography/TOE parameters’ predictive ability for SS categories 3 (high risk) and 1 (low risk). The trans thoracic echocardiography (TEE) parameter (TELPD = TEE left main coronary artery peak diastolic velocity in cm per second (LMCA)) was found to be successful in indicating SS category 3 (high) reliably, with 72.4% sensitivity and 71.8% specificity for a cut-off value of 84.5. The Two TEE parameters were found to be successful in indicating SS category 1 (low) reliably, the first one was TELD (TEE_LMCA, diameter in mm) with 77.8% reactivity and 70.9% precision for a cut-off value of 3.81, the second one was TERD (TEE_right coronary artery, diameter in mm) with 71.1% sensitivity and 63.6% specificity for a cut-off value of 2.72. Conclusion: This study shows the use of TOE in evaluating atherosclerosis and the angiographic relationship between it and coronary artery disease (CAD), providing insightful information for better diagnostic and prognostic assessment in patients with CAD.
