Three months of colchicine cuts early AF recurrence ~62% vs placebo after PVI.
Why the study?
Proinflammatory processes induced by AF ablation therapy have been implicated in postablation arrhythmia recurrence, but the potential of colchicine to reduce early AF recurrence after pulmonary vein isolation remained to be tested.
Does colchicine reduce early atrial fibrillation recurrence in patients with paroxysmal AF after pulmonary vein isolation?

Does colchicine reduce early atrial fibrillation recurrence in patients with paroxysmal AF after pulmonary vein isolation?
Effect estimate: OR 0.38 (95% CI 0.18 to 0.80)
Absolute Event Rate: 16% vs 33.5%
A 3-month course of colchicine significantly reduces early atrial fibrillation recurrence after pulmonary vein isolation, an effect associated with decreased inflammatory mediators.

May justify short-term colchicine post-PVI in paroxysmal AF; challenges consensus against benefit.
Deftereos et al. (2012) conducted an RCT in paroxysmal atrial fibrillation (n=161). colchicine vs. placebo was evaluated on recurrence of AF in the 3-month follow-up (OR 0.38, 95% CI 0.18 to 0.80). A 3-month course of colchicine significantly reduced early atrial fibrillation recurrence after pulmonary vein isolation compared to placebo (16% vs 33.5%; OR 0.38, 95% CI 0.18-0.80).
