ABSTRACT Root Cause Analysis (RCA) is a powerful tool for any type of investigative work. This article provides an overview of RCA, presents an in-depth walkthrough of an RCA process, and outlines the currently available RCA software and tools. A solid understanding of RCA, how to implement RCA, and the benefits it can bring to an organization will improve any investigative process, provide continuity amidst frequent personnel turnover, and build confidence in the quality of the solutions implemented.
